Fees

¹ Initial phone consultation is available to any new clients who would like to learn if OT would be appropriate for their situation.

² Evaluation includes a 60-minute session of informal and/or formal assessment and goal setting. If further assessment is required, it will be discussed during the initial evaluation session, indicating how much time would be required and to determine if a formal report is required. Further assessment and report writing will be charged at the hourly rate to the nearest half hour.

³ An initial evaluation must be completed before a treatment session can be booked.

⁴ Ergonomic assessment fees vary based on needs and can include a 60-90 minute onsite workstation evaluation, verbal and written onsite recommendations or a formal report, and a follow up phone call with the worker if needed.

⁵ LSVT BIG program fee includes 16 x 60-minute intervention sessions, a 90-minute re-evaluation session and a 30 minute session to review results. A participating client must have completed an Initial Evaluation (paid separately) first which may take between 90 and 120 minutes over several sessions to complete.

⁶ Cancellation fee will be applied to any appointment cancelled without 24 hours advance notice

⁷ Travel flat rate is required for home visits or other visits not at the clinician’s office, to account for the time not available for other appointments and costs of mileage.
